How Do I Request Information About ADA Service?
You need to contact the City of Minot at 857-4140 or Souris Basin Transportation at 839-7433 and request an ADA eligibility form. This form must be filled out and returned to the Minot Commission on Aging for review. If you cannot come in to fill out the form, an outreach worker will be sent to assist you. These forms will be processed as quickly as possible and upon approval, you will be issued a picture ID. ALL RETURNED FORMS ARE CONFIDENTIAL.
How Soon May I Receive Service?
Service may begin as soon as the next day after a request. ALL requests for service must be made one day in advance of your appointment to allow the dispatcher to schedule a pickup time and a return time. For your convenience, an ADA eligibility form can be sent with the driver.
Must I be Permanently Disabled to Receive Service?
No. Should you require ADA transportation because of a temporary disability, call and request an ADA eligibility form, and fill out the necessary information. Upon approval service will be provided for the length of time necessary.
What If I Need ADA Service Only During Cold Weather?
Contact the City of Minot or Souris Basin Transportation, ask for an ADA eligibility form, and fill out the necessary information. Upon approval of your request, arrangements will be made for your transportation needs.
What Time Does Service Start and End?
Complementary service starts at 7:30 A.M. and ends at 5:30 P.M.
What Days Can I Expect To Get ADA Transportation?
Service is provided Monday through Friday of most weeks. The only holidays that service is provided are the following: Columbus Day, Veteran's Day, and Washington's Birthday.
How Far in Advance May I Schedule Appointments for Rides?
Appointments may be made a minimum of one day in advance to a maximum of 14 days.
Can I Make A Request for a Ride After Regular Hours?
Souris Basin Transportation has installed an answering machine to accommodate calls after hours and also on weekends. Call 839-7433 to request service.
What If I'm Hearing Impaired?
To accommodate those who need assistance Souris Basin Transportation has installed a telecommunication display Device (TDD). Call 852-3390 to request service.
What if I Need A Personal Aid to Travel With Me?
If you require a personal aid to assist you in your travels, this will be addressed when filling out your ADA form. Upon ADA form approval, your personal aid may travel with you at no charge. Friends or relatives do not qualify as a personal aid unless they are listed on the ADA form. Should someone wish to travel with you other than your personal aid, they must pay the regular fare (same as your cost).
Must I Give the Reason For My Trip?
No. The only information required is pickup time and your destination.
Who Do I Call if I Have A Problem With ADA Eligibility?
Should you be denied eligibility or should you lose eligibility for ADA transportation, the City of Minot would grant a hearing. This appeal must be submitted within 60 days of the denial.
Who Do I Call if I Have Trouble With Scheduling Or Busing?
Contact the Dispatcher at 839-7433. If the problem cannot be resolved, contact the City of Minot Bus Department at 857-4140. The City of Minot will only address the problems of passengers that are ADA APPROVED.
What if I Have Someone Who is Coming to Visit and Needs ADA Transporation?
If the person coming to visit is certified ADA and has ADA identification, that person is allowed to use our system for 21 days. After that time, this person will be required to certify with the City of Minot.
